A person receiving funding from The Developmental Disability Administration through the New Directions Wavier or Community Pathways Waiver in a Community Supported Living Arrangement may be required to have a Nurse Case Manager/Delegating Nurse.

1.What does a Nursing Case Manager do?
Health Link Nurse Case Mangers work directly with you and your team of support to first, decide what amount of nursing case management support you are looking for. We then tailor make a plan for you based on your needs and wants. Our nurses can assist you with developing a life wellness plan, develop a plan to manage a chronic illness and/or assist you to find healthcare resources you need. Typically nursing case management contracts fall in to one of the following categories.
2.What if I am self administering my own medication?
Health Link nurses are available to provide nursing consultation with the added service of supporting individuals achieve their goal of safely self administering their own medications. Individuals may utilize an array of organizational and prompting supports to maximize independence while maintaining safe medication administration.
3.What if I rely on support staff to administer my medication?
When individuals rely on the support of others to receive medication and proper health care treatments they require the services of a Nursing Case Manager/Delegating nurse.
The Delegating Nurse makes regular home visits no less than every 45 days. Is on call for consultation and supervises support staff who adminster medications. Support staff in this role must be certified with the Maryland Board of Nursing as a Certified Medication Technician and work at the Delegation of the Registered Nurse.
4.What if I only take over-the-counter medications?
Over-the-counter mediation, herbal remedies, and vitamin suppliments are subject to the same regulations as perscription medications.
5.What is Nursing Consultation?
Nursing Consultation services are the most basic of Nursing Case Management Services. With consultation, your nurse is available 24/7 for assistance via telephone. Typical consultation needs include resource and referral for other health care providers, assistance in understanding medical documents such as test results, negotiating health care insurance and regulations governing healthcare.
